Muse/Model Nomination Form

Please provide the information requested to be considered to be one of our muse/models for the Hidden Faces project.
All participants in the project must be willing to sign a waiver of rights for photographs taken during the session.
You need to be available September 28th and/or 29th. Your session will take between 4-6 hours.
